Variable Insulin Sensitivity

As some of you know, I went on testosterone under medical supervision a little over two months ago. I also got a nast cold. My Lantus doses had been averaging around 15 units, but climbed up up up. For two months, I took 20-30 units of Lantus daily. For the last week, I've been lowering the Lantus dose almost every day, and still dropping dropping dropping.
On Friday I took 21 units of Lantus. On Saturday I took 18. On Sunday I took 15. On Monday I took 13. On Tuesday I took 11. Since I still was dropping way too much overnight, this morning I took 10. If I wasn't eating, or if I was being really active, I wouldn't be so confused. But I'm eating; probably had 300 carbs for breakfast today. And I am active, but not really really active. I dunno. I'm taking less Novolog than usual, but that's only partly because of a ratio change; with my blood sugar dropping quickly, I can get in a lot of carbs without Novolog.

Why is my insulin sensitivity so variable? I think if I followed the advice of some and waited three days to see full effects of Lantus changes, my blood sugars would be way way worse.

My little brother has epilepsy and recently it got more intense so we're checking his blood sugars. This morning's fasting was 88. I am guessing that the nutritionist is thinking about a ketogenic diet, but I'm not certain. I don't know of any other reason to check an epileptic's blood sugar, except suspicion of diabetes. This is the brother who refused to have his blood drawn when my mother took him in for TrialNet, so we don't know if he has antibodies or not.
